We take the intakes you can't place elsewhere.
Licensed skilled nursing for spinal cord injury patients, complex cases, injured workers, and veterans, with documentation built for case managers and carriers.
Getting started is simple.
Case managers, discharge planners, and physician offices can call (619) 220-7600, fax referral documents to (619) 872-4865, or request a callback below.
Initial contact
Call us or request a callback, and fax the physician order and referral documents to (619) 872-4865.
Case review
We review the referral against the patient's clinical needs and coverage.
RN assessment
An RN completes an in-home evaluation and builds the plan of care.
Care begins
A nurse is assigned, with documentation built for the referring case manager.
Built for how case managers actually work.

- RN-directed plans of care for complex, long-term, and post-surgical cases
- Clear, consistent documentation for case-manager review
- Direct communication with the referring case manager
- A single point of contact throughout the case
Workers' comp and case managers
Injured workers often need skilled nursing during recovery, and carriers need clear documentation to go with it. Workers' compensation referrals follow the same steps above.
- Skilled nursing during recovery: wound care after surgery, medication management, mobility support
- Documentation formatted for carrier and adjuster review
- Send the claim number, the adjuster's contact details, and the authorization with the physician order
Veterans and VA coordination
Firstat can work with VA case managers on care for veterans. When a veteran's care may involve VA benefits, call us and we'll help work out the right path before care begins, sometimes alongside a partner organization.
- Skilled nursing at home for veterans, including spinal cord injury and long-term complex cases
- Help coordinating with the VA, sometimes through a partner organization
- Send the physician order, any VA paperwork you have, and your direct line
What to send
